Requirements:
For Burial Assistance:
- Photocopy of Death Certificate (claimant should be any immediate member of the family of the deceased)
For Medical Assistance:
- Doctor’s prescription/Medical Certificate /Hospital Bill
Food and Transportation Assistance:
- Police Blotter, in case client is a victim of pickpockets
- Certificate of Indigency from the Barangay, in case of food assistance
